Our duties
American Graft Hair Restoration is required by law to maintain the privacy of your protected health information (PHI), to give you this notice of our legal duties and privacy practices, and to abide by the terms of the notice currently in effect.
How we may use and disclose your health information
- Treatment — for example, sharing information among the clinical team involved in your care.
- Payment — for example, confirming coverage or processing payment for services, including with a financing partner you have chosen.
- Health-care operations — for example, quality review and training, under strict access controls.
- As required by law, or to prevent a serious threat to health or safety.
Uses beyond these — including most marketing uses and any sale of your information — require your written authorization, which you may revoke at any time. Publication of your photographs in our results gallery happens only with your separate, written media-release consent.
Your rights
- To inspect and receive a copy of your health record.
- To request corrections to your record.
- To request restrictions on certain uses and disclosures.
- To request confidential communications (for example, only calling a specific number).
- To receive a list of certain disclosures we have made.
- To receive a paper copy of this notice on request.
Complaints
If you believe your privacy rights have been violated, you may complain to us using the contact details on our contact page, or to the U.S. Department of Health and Human Services. You will never be penalized for filing a complaint.
